Don't know exactly how, but I died once and got a respawn that went on for 5 minutes, I was kind of disappointed, so I got a video of it, but then on another playthrough I died three times with out any trouble.
In the infinite respawn I could do NOTHING and I had to use task manager to close the game down completely. Kind of disappointing and that was my first death so I wasn't aware anything odd was going on.
